In the midst of the treatment that lead to liver failure, the specialist treating Holmes apparently disappeared and put nothing in the patient's file that said he was being treated for liver disease.

The report did not specifically say the hospital killed Holmes because they said his final hospitalization in December was "timely," despite the fact his illness was caused by the hospital.
Mark Magill, the center's medical director, took no responsibility for Holmes' death--despite the evidence in the inspector general's report.
“The veteran’s issues were appropriately addressed at each encounter, including medication adjustments, emergency room treatment and IV fluids, and hospitalization when appropriate," Magill told the Denver Post.
A social worker close to the case quit in disgust over the treatment Holmes received at the hospital and is appealing the inspector general's report because they don't feel the report was accurate enough in what they felt was the negligence of the hospital.
